Dear Customers, Please thoroughly read these terms.

-On The Road Maintenance Service and Shop Labor that I provide will be as listed.
         -Hourly rate will be $50.00 per hour between the hours of 7:00 AM and 5:00 PM.
         -Hours from 5:00 PM to 12:00 Midnight on weekdays as well as anytime on Saturday
                  will be charged overtime at a rate of $75.00 per hour.
         -Hourly rate starts when I leave my shop and ends when I return.
         -Long distance traveling may include charges such as lodging, highway tolls, and meals.
         -No work will be done on Sunday.

-Emergency Road Maintenance Service will be as follows.
         -Jobs that can't wait to be scheduled and must be done on short notice will be charged
                  $75.00 per hour regardless of what time of day it is.
         -Hourly rate starts when I leave my shop and ends when I return.
         -Long distance traveling may include charges such as lodging, highway tolls, and meals.
         -No work will be done on Sunday.

-Fuel Surcharge will be issued when gas is over $2.00 per gallon.
                   If gas is between $2.00-$2.50, Surcharge will be $0.45 per mile round trip.
                   If gas is between $2.50-$3.00, Surcharge will be $0.50 per mile round trip.
                   If gas is between $3.00-$3.50, Surcharge will be $0.55 per mile round trip.
                   And so on.

-Small Machine Deposit of $50.00 will be required for small woodworking equipment brought to the shop.
                   If machine is repaired, the deposit will be applied to the invoice.

                                                             Payment Policy

-For New Customers that don't have credit approval with Ken-B-Done Mfg. Co.
         -Payment for all parts required for job prior to ordering.
         -Payment of invoice balance in full upon completion of job, unless other arrangements
                  have been made.

-For Current Customers that have credit approval with Ken-B-Done Mfg. Co.
         -All invoices term net 30 days.
         -Large part orders exceeding $500.00 will be paid prior to ordering

-Terms For Quoted Jobs will be as follows.
         -50% Down payment and P.O. Number before starting job.
         -30% Payment upon completion of the job.
         -20% Balance NET 30 days.
         -Approved personnel must sign Job Proposal Sheet. Any quoted job changes must be approved in writing.

         Payment terms are net 30 days from date of completion of job and must be adhered to.  A late fee of 2% per month (24% per year) will be charged on past due invoices.  If account remains past due for more than 60 days such past due accounts are subject to collection.  Applicant agrees to pay all cost and expenses of collection, including reasonable attorney's fees incurred by Ken-B-Done Mfg. Co. Inc. in connection with collection.
         As owner or duly elected officer or agent of the previously stated corporation, the undersigned warrants that the information herein given is correct and requests that standard credit terms be extended by Ken-B-Done Mfg. Co. Inc. to Applicant based on this information. I authorize our references to release information directly to Ken-B-Done Mfg. Co. Inc. for such verification.
         Applicant acknowledges that he has read the Terms and Conditions set forth above, is familiar with the contents of suck provisions, and agrees to said terms and the application of such provisions..
         Must be signed by owner, corporate officer, or agent having authority to bind the Applicant.
